Independent Care Solutions specialises in providing high-quality supported living services and accommodation for vulnerable adults and young people aged 16 years and above, as well as domiciliary care services for individuals in the community. We are looking for a dedicated Care Coordinator to support the delivery of outstanding care services, ensuring clients receive person-centred, safe, and effective care.
Role Overview
The Care Coordinator plays a vital role in ensuring the smooth delivery of care services. This includes managing staff schedules, liaising with service users and families, maintaining compliance with regulations, and supporting the care team. The role requires excellent organisational skills, strong communication abilities, and a commitment to high-quality care.
Key Responsibilities

Care Planning & Coordination:
- Develop and maintain individual care plans, ensuring they are tailored to service users’ needs.
- Allocate care staff to service users based on skills, experience, and location.
- Adjust schedules and respond to last-minute changes in care arrangements.

Staff Management & Support:
- Supervise and support care staff to ensure high standards of service delivery.
- Conduct spot checks, supervisions, and performance reviews.
- Identify training needs and ensure staff have up-to-date qualifications.

Client & Family Liaison:
- Serve as a point of contact for service users, families, and external professionals.
- Address concerns, complaints, and feedback to improve care services.
- Ensure service users are satisfied with their care and support.

Compliance & Safeguarding:
- Ensure all care services comply with CQC regulations and company policies.
- Maintain accurate records of care delivery, risk assessments, and incident reports.
- Uphold safeguarding procedures and report concerns promptly.

Administrative & Operational Support:
- Maintain electronic and paper-based records, including care logs and staff rotas.
- Coordinate staff recruitment, inductions, and training sessions.
- Ensure effective use of resources and support financial efficiency in care delivery.
Requirements
- Previous experience in a Care Coordinator role or a similar position in domiciliary care or supported living.
- Strong understanding of CQC regulations and compliance.
- Excellent organisational and problem-solving skills.
- Strong IT skills, including experience with care management systems.
- A compassionate and professional approach to service users and staff.
- NVQ Level 3 in Health & Social Care (or willingness to work towards it).
- A valid UK driving licence and access to a vehicle is critical.
