Special Requirements:

This placement is for working from home and within GP Surgeries in the Southend area.
Conducting mental health assessment, of patients experiencing mental health issues. Good communicator.
Able to sign post, provide advice and guidance, co-production, risk assess, formulate care plan for home and may include some home visits.
The role will be delivered as a hybrid model, including being based on GP practices
Responsible for the line management of identified members of the service.
Meet the requirements for revalidation as set out by the NMC.
Experience of working within a Mental Health setting, Ability to inspire confidence in the MDT, service users, carers and junior staff.
Venepuncture trained, trained to deliver Physical Health screening
Current driving licence, ability to travel across sites efficiently and across Trust boundaries to attend meetings, etc
